Pasireotide (Signifor / Signifor LAR): what the evidence says

At a glance

What it is: a somatostatin analogue that binds SSTR5 strongly, plus SSTR1, 2 and 3. R1 FDA approved Signifor (NDA 200677) and Signifor LAR (NDA 203255). R2, R3

What the evidence says: in Cushing's disease, 12 of 82 and 21 of 80 patients reached normal urine cortisol at month 6. R1 In untreated acromegaly, it controlled disease in 31% vs 19% with octreotide LAR. R4 It also helped patients not controlled on older drugs (PAOLA). R5

Biggest risks: high blood sugar: 118 of 162 Cushing's patients had hyperglycemia events. R1

Rating

Human Human evidence Strong: phase 3 RCTs, n = 162 to 358 R1, R4, R5
Animal Animal / lab evidence Not reviewed
Regulatory FDA-approved R2, R3 · not on FDA's 503A or 503B bulk-substance category lists and not on FDA's compounding safety-risk page R6, R7, R8 · not named by WADA R9
Established human dose Yes, label dose

Evidence verdict: Cushing's disease: Strong (FDA-approved; works in a minority) · Acromegaly: Strong (FDA-approved) · Blood sugar harm: Serious concern (most patients)

About the evidence: phase 3 trials with clear endpoints. The Cushing's response rate is modest, and high blood sugar is common.
